Pembroke Public Library is an authorized Passport Acceptance Facility and accepts U.S. passport applications by appointment; make an appointment by calling the Library at 781-293-6771.
How to get your passport
Minimum requirements for applying for a U.S. passport:
PLEASE NOTE: If you have an existing passport in good condition that was issued in your current name within the last 15 years and after your 16th birthday you must renew by mail using Renewal Form DS-82.
- DS-11 passport application form. This form can be downloaded online or picked up at the library. Forms must be printed out one-sided and filled out in black ink. Make sure you do not sign the application before coming in for your appointment.
- Proof of citizenship. An original or certified full copy of your birth certificate or certificate of naturalization, plus a photocopy. Birth certificates must have a seal and include the applicant's full name, date and place of birth, both parents' names, and the date of record. Expired U.S. passports are also acceptable proof of citizenship for applicants age 18+.
- Valid driver's license or government photo ID. Include a photocopy of both sides (can be on two pieces of paper). Applicants with out-of-state licenses and state ID cards must present a second form of ID. See the Department of State website for a full list of accepted identification documents.
- Passport photo. A 2" by 2" passport photo taken within the past six months. Passport photos may be taken at any CVS, Walmart, Walgreens, or AAA.
- Payment. The application fee must be paid by check or money order made out to the U.S. Department of State. (For current fees see travel.state.gov.) A facility acceptance fee of $35.00 per application is payable by cash or check made out to Pembroke Public Library. This fee is mandated by the Department of State and is the same at every acceptance facility.

Minor applicants (under age 16):
*If only one parent is available to accompany a minor under age 16, please refer to the front of form DS-11 and read the requirements for the use of notarized form DS-3053 to be submitted with the passport application. Please note that a photocopy of the non-appearing parent's ID must be submitted with DS-3053.
- To obtain or renew a passport for a minor under the age of 16, the minor and both parents* must all be present. (Please note that for applicants age 17-18, it is strongly encouraged that one parent accompany the applicant to show consent by signing the application with them.)
- Both parents must present a valid driver's license or government photo ID plus a photocopy of both sides.
- The original or certified copy of the minor's birth certificate showing both parents' names is required even if a passport was previously issued to the minor.
- If the minor has previously been issued a passport and it is still valid, it must be sent in with the application.
